First, we would like to thank you for trusting us with your healthcare. We are glad you have decided to follow-up with us via our new telehealth service! Before your visit, here are a few things you need to know:
You will need a phone, laptop, tablet, or desktop computer with a microphone and camera. You will not need to create a login or account in order to meet us in our "virtual waiting room", however you will need to have Safari, Firefox, or Google Chrome web browser.

*Our staff will call you at the time of your scheduled appointment to verify insurance, confirm that we have received a copy of any lab work that was to be done prior to this visit, update your medication list, and confirm update of any insulin pump/ CGM (if applicable).
You are enter the below "waiting room" at your scheduled appointment time. When your provider is ready for your visit, they will show up on your computer. As with a typical waiting room, you may anticipate some wait. We will be doing the best we can to make sure that you are seen in a timely manner.
If you are being seen for diabetes management, then it is highly recommended that you send us a copy of your blood glucose logs via our online portal (you may attach a photo of them to a message sent to our practice/providers). Click here to login to the health portal to send a message.
If you have an insulin pump and/or continuous glucose monitoring (CGM) device, then it is recommended that you upload these devices the day before your visit. **

**Some insulin pumps and CGM's should already be sharing data with our practice, in which case you will not have to do anything on your part (Omnipod5, Tslimx2- if using T:connect mobile app, Dexcom Clarity- if using mobile app, and freestyle libre- if using mobile app). Devices that will need downloaded include those with readers (libre 2-if using reader, Dexcom G6 and G7- if using reader, Tslim x2- if not using mobile app, Omnipod Eros and DASH).
