Our Practice
Indiana Diabetes & Endocrinology was established in 2019 in Indiana, PA.

Our small town endocrine medical practice specializes in diagnosis and treatment of various diseases of the endocrine system, including diabetes, thyroid disorder, thyroid cancer, parathyroid disease, osteoporosis, and low T (just to name a few).

Our Providers
Mushtaq Syed, MD
​
Dr. Syed is an endocrinologist who is board-certified in endocrinology, diabetes, and metabolism by the American Board of Internal Medicine.
​
Although new to this practice, he is not new to our area. Dr. Syed has been proudly providing care to patients in our area since 2008. He holds hospital privileges at IRMC.
He received his medical degree from Liaquat Medical College in Jamshoro, Pakistan.
​
Following his education, Dr. Syed completed a medical and surgery internship and residency at Liaquat Medical College. He then completed an internal medicine residency at Albert Einstein College of Medicine in Bronx, NY. His post-graduate training involved four years of clinical and research fellowship at the University of Pittsburgh School of Medicine in the field of endocrinology and metabolism.

He holds professional memberships with the American Association of Clinical Endocrinologist, the Endocrine Society of America, and the American Thyroid Association.

Annette Kubalek, CRNP
​
Annette Kubalek is a certified registered nurse practitioner who is board certified as a Family Nurse Practitioner from the American Academy of Nurse Practitioners.
​
Annette has been seeing patients as an FNP in Endocrinology in our area since 2016. She has hospital privileges at IRMC.
She received her Bachelor of Science degree in Nursing from Indiana University of Pennsylvania and earned her Master of Science degree in Nursing from Wheeling-Jesuit University in Wheeling, W.V.
​
She is Board Certified in Advanced Diabetes Management through the Association of Diabetes Care & Education Specialists.
​
Annette holds professional memberships with the American Association of Nurse Practitioners, the American Association of Diabetes Education, and the Endocrine Nurses Society.
